The MLS Library copy consists of typed pages, cut into two halves and bound in a wrapper made of cardboard and bookbinding(?) tape. This appears to be the original Church Army binding, and its apparent late date (the CA dress has the London postal district W3, i.e. after 1916) suggests that no printed copy was published.
